Basic Plan

• Prescription Drug Discount: Your membership entitles you to discounts of up to 40% off regular retail prices on prescription medications. If for any reason, your regular price is already less than this across the board-discounted price, you will always be charged the lower price.
• Dental Plan: Members will save on almost all dental care expenses through the largest independent network of dental providers nationwide. Simply present the membership card to any one of the more than 24,000 providers (including specialists, where available) to receive discounted rates on dental procedures.
• Vision Plan: As a program member, you’ll save 20% to 60% on eyeglasses, contact lenses and other retail eyewear items. You can use your program identification card at any local participating optical service provider from our nationwide network of more than 13,000 eye care professionals.
• Diabetic Supplies: Matrix Medical Supply provides savings on glucose test strips, lancets, meters and syringes.

Custom Plan

• The Custom Plan offers all of the same benefits as the Basic Plan, with the addition of the enhanced Prescription Drug plan outlined below:
• Prescription Drug Discount: Preferred brand and generic drugs will be offered at $10/$20/$40 tiers. Non-preferred medications will be discounted at a negotiated discount.
• Mail Service Pharmacy: Members can get up to a 90-day supply of medication delivered to home or work.
